Child Abuse Clearance Form

All managers and assistant coaches must complete this form and send it in to be eligible to coach.

 DeHaven will reimburse you the $10.00 fee when your form is returned to the commissioner of your age level.

This is mandatory to protect our children.

Registration fees are as follows:

Boys and girls ages 4-6           $60.00                            Girls slow pitch ages 7-10       $85.00 

Girls slow pitch ages 11-18     $90.00

Girls fast pitch ages 9-12        $90.00

                                                                                     Girls fast pitch ages 13-18      $105.00

Boys baseball ages 7-8            $85.00

Boys baseball ages 9-10          $85.00

Boys baseball ages 11-12        $90.00

Boys baseball ages 13-14        $125.00

Boy's baseball ages 15-16        $125.00  We will accept registrations but cannot guarantee we will have enough to field a team. If we are unable to do so we will issue refunds.

The family maximum registration fee is $195.00.  (This excludes Federation, Colt League, Pony League and Fast Pitch Softball, when double rostering with Slow Pitch Softball.). Late registration will be subject to a $10.00 late fee per player.
